Collection Instructions:
A pink (EDTA) is the container of choice for blood bank collections at FSH, FRH, FNMC, and Maple Grove.

Strict specimen labeling requirements must be followed for ABO/Rh testing compatibility testing. The patient's full name and identification number on the specimen label MUST EXACTLY MATCH the name and identification on the request form. The specimenl abel must include the following information.
  1. Patient's first and last name
  2. Patient's identification number
  3. Date and time of specimen collection
  4. Inititals of the person collecting the sample, indicating that the patient's identity has been verified.

For patients that do not have an historical ABO/Rh type on file, the sample must be either
  1. Collected using VeriSafe, or
  2. Identified by two persons who have each identified the patient, or
  3. Collected a second time for repeat testing
