Collection Instructions

Specimen:Blood
Optimal Volume:2.5 mL
Minimum\Peds Volume:0.6 mL; pediatric patients only: 0.4 mL

Trough levels: Collect immediately prior to next dose. Peak levels: collect 2 hours after the end of IV infusion; 3-4 hours if decreased renal function.
Causes for Rejection:Collection in a gel tube.


Processing and Shipping

Specimen Processing:Centrifuge and aliquot within 2 hours of collection 1.25 mL, 0.2 mL minimum (adult) 0.1 mL minimum (peds). Store in refrigerator.
Shipping Instructions:Ship at refrigerated temperature.
Stability:Roche: 48 hours at room temperature; 14 days refrigerated; 12 months frozen. Can be frozen and thawed up to 5 times.

Other sites: Greater than 25 mg/L
Therapeutic Range:Roche: Trough: 10-15 ug/mL; Peak 20-40 ug/mL
All others: Trough: 8-20 mg/L; Peak: 20-50 mg/L (traditional dosing)
Use:Monitor therapeutic drug level. Half-life is 4-10 h (adult); 2-3 h (child); 6-10 h (neonate).
