Methodology:Amplification of DNA and detection by capillary electrophoresis.

Collection Instructions

Specimen:Blood, bone marrow, fresh frozen tissue or fixed tissue.
Optimal Volume:10 mL blood; 5 mL bone marrow; 5 mm x 5 mm x 5 mm fresh tissue, embedded tissue (sufficient for 60-100 five micron sections)
Minimum\Peds Volume:5 mL blood; 2 mL bone marrow; 2 mm x 3 mm x 3 mm fresh tissue
Container:Yellow (ACD, Solution A) tube available from laboratory (Purple (EDTA))
Collection Instructions:
  • Peripheral Blood: Collect 10 mL peripheral blood in yellow (ACD) tube; alternate container: purple (EDTA) tube.
  • Bone marrow aspirate: Aspirate ACD (or EDTA) solution into syringe from ACD (or EDTA) tube; collect 5 mL bone marrow into syringe. Deliver in syringe.
  • Fresh tissue: Collect in sterile container; deliver to the laboratory within 1 hour, freeze specimen (liquid nitrogen or dry ice) and transport to the laboratory frozen (-70 C); or place in RPMI and transport same day at 4 C.
  • Paraffin embedded tissue: Indicate fixative used.
Causes for Rejection:Clotted or frozen blood or bone marrow; necrotic tissue, incorrect anticoagulant, specimen more than 5 days old, clinically not indicated, B cells by flow cytometry rare to absent, decalcified tissue.
Shared blood or bone marrow samples will not be accepted unless sent to Molecular first. Contamination will occur on automated hematology analyzers.

Processing and Shipping

Specimen Processing:Store at room temperature. Do not freeze.
Shipping Instructions:Ship blood, bone marrow, and fixed tissue at room temperature to arrive within 24 h. Ship fresh tissue at RT to arrive within 1 h, in RPMI to arrive same day, or ship on dry ice to arrive within 24 h.
Stability:After hours and weekends, store blood and bone marrow at RT; if not received in RPMI, store tissue at -70 C. Tissue must arrive within 1 hour of collection, be frozen at - 70 C or received in RPMI,

Reference Range:Results are reported as negative (germline only or no rearrangement noted) or positive (clonal rearrangement indicating clonal cell populations present). Correlations with immunophenotypic, histopathologic, and cytogenetic data will be included with the interpretation of the molecular data when available.
Use:Evaluation for clonal rearrangements of the immunoglobulin heavy chain as an adjunct in diagnosing lymphomas and leukemias of B-cell lineage.
