Order Instructions:When ordering both a culture and a gram stain in Epic, use Epic Code 03046000096 for an inpatient or 04046000096 for an Ambulatory patient.
Synonyms:Corneal Ulcer Culture
Test Includes:Culture for aerobic flora plus Haemophilus species. Culture for fungus included with corneal ulcer kits or if specially requested. If Neisseria gonorrhoeae is suspected, a GC kit must be ordered from the Micro Lab; this includes media and the CO2 transport container. If AFB (Acid Fast Bacilli) is suspected, please request a vial of sterile saline from Micro Lab to inoculate.
Turnaround Time:Performed 24 hours/day; preliminary results are reported within 24 hours; physician is contacted if positive. Results are reported within 7 days if negative.

Collection Instructions

Specimen:Swab or scrapings of cornea
Container:Plastic leakproof container, sterile
Collection Instructions:Swab: pass moistened swab two times over lower conjunctiva. Avoid eyelid border and lashes. (Culture these separately in a similar fashion if indicated.) Scrapings: Use local anesthetic and platinum spatula. Rub spatula with scrapings gently over small area of agar plate media. Inoculate additional scrapings into broth tube media. If too dry, use a very small amount of sterile water. Deliver to laboratory immediately.
Causes for Rejection:GC not received in CO2 transport container.

Processing and Shipping

Specimen Processing:Send to lab immediately. If not possible, store at room temperature.
Shipping Instructions:Ship in CO2 transport container if Neisseria gonorrhoae is suspected. Ship at room temperature. Must arrive within 24 hours.
Stability:Considered a stat specimen. (OD = right eye; OS = left eye).

Reference Range:No growth.
Use:Isolate and identify potentially pathogenic organisms. Normal ocular flora may include: Corynebacterium species (diphtheroids); coagulase negative Staphylococcus; Neisseria species; Moraxella species; nonhemolytic streptococci. Abnormal ocular flora includes: Haemophilus influenzae; Streptococcus pneumoniae; Staphylococcus aureus; Bacillus species; Pseudomonas aeruginosa; Neisseria gonorrhoeae; some saprophytic fungi.
