Iron Binding Panel: Iron Transferrin Iron Binding Capacity - Grand Itasca Only

Abbrev Code:GHFEBC
Order Code:LAB4124Order Name:Iron Binding Capacity
Synonyms:ITS
Methodology:Grand Itasca: Calculation based on iron and transferrin levels
CPT Codes: 83550 x1
Turnaround Time:Performed 7 days/week, except clinics which are performed Mon-Fri.
Special Instructions:Falsely elevated iron levels will result if the patient has received Imferon within the past two months.

Collection Instructions

Specimen:Blood
Optimal Volume:2.5 mL
Minimum\Peds Volume:0.6 mL
Container:Green (lithium heparin, gel)
Alternate Containers: Green (lithium heparin, no gel), Red (no gel), Red or gold (gel)
Collection Instructions:A fasting AM specimen is preferred because of diurnal variation. Collect sample before patient is given therapeutic iron or a blood transfusion. Blood transfusion can transiently (<24 h) increase serum iron concentration and transferrin saturation (but not total iron binding capacity), particularly if the blood has been stored for a longer period (Blood 2011 Dec 15; 118(25):6675-82; Arch Pathol Lab Med 1993 Jun; 117(6):(622-4). Calculation for Transferrin Saturation: (serum iron/TIBC) x 100.
Causes for Rejection:Hemolysis

Processing and Shipping

Specimen Processing:Centrifuge and aliquot 1.25 mL, 0.2 mL minimum. Store in refrigerator.
Shipping Instructions:Ship at refrigerated temperature.
Stability:7 days refrigeratedd; 6 months frozen
Test Performed at or Referral Lab Chemistry  (Grand Itasca)

Interpretive

Reference Range:Grand Itasca (All ages):
    Calculation
Iron 50-212 μg/mL N/A
Transferrin 203-362 mg/dL N/A
IBC 245-400 ug/mL (Transferrin)*(1.4)
Iron Saturation 20-55% (iron)/((Transferrin)*1.4)*100
UIBC (unsaturated) N/A ((Transferrin)*1.4)-(Iron)
