Collection Instructions

Specimen:Blood placed in special collection tube within 10 minutes of collection.
Optimal Volume:2 mL
Minimum\Peds Volume:1 mL
Container:Green (sodium heparin, no gel) on ICE, Pyruvate collection tube
Collection Instructions:
Special handling required.
  • Collect 1-2 mL of blood into a green (sodium or lithium heparin) tube.
  • Immediately after blood is drawn, add exactly 1 mL whole blood to a chilled pyruvate collection tube containing 2 mL 8 percent (w/v) perchloric acid
  • Mix thoroughly for 30 seconds by inverting 10-15 times and place in ice bath for 10 minutes. Deliver to the lab immediately.
  • Processing must occur before the sample is transported between campuses. Immediately centrifuge mixed specimen at 1500 x g (RCF) for 10 minutes. Decant 2 mL supernatant to an ARUP Standard Transport Tube and freeze immediately (min 1 mL).
   
Patient Preparation:Patient should be fasting and at complete rest. Patient should avoid any exercise of the arm or hand before or during collection. Draw the specimen without the use of a tourniquet or within three minutes of applying the tourniquet, but before releasing the tourniquet.
Causes for Rejection:Room temperature specimens. If less than 1 mL of blood is added to collection tube, pH of the supernatant will be too low for testing.


Processing and Shipping

Specimen Processing:Special handling required. Processing must occur before the sample is transported between campuses. Immediately centrifuge mixed specimen at 1500 x g (RCF) for 10 minutes. Decant 2 mL supernatant, 1 mL minimum, into an ARUP Standard Transport tube and freeze immediately. (min: 1 mL)

Interpretive

Reference Range:0.030-0.107 mmol/L (venous blood)
Limitations:If less than 1 mL of blood is added to collection tube, pH of the supernatant will be too low for testing to be performed.
Use:Evaluate diabetic ketoacidosis and other metabolic disorders.
