| Collection Instructions: | Draw using a 21 G or larger needle after any EDTA tubes were drawn (order of draw); ensure there is no backflow of tube contents. Tube must be filled completely and inverted 8-10 times.

| Patient Preparation: | AlloMap Test is indicated for heart transplant recipients 15 years of age or older and at least 2 months (greater than or equal to 55 days) post-transplant at the time samples were collected. |
