DNA Marker, Pre BMT Engraftment, Recipient Testing

Abbrev Code:RCENFT
Order Code:LAB4728Order Name:Pre BMT Polymorphism Determination Recep
Order Instructions:For donor samples, use code: DNENFT, LAB4727
Synonyms:Bone Marrow Transplant DNA Marker; DNA Polymorphism; Pre-Transplant Chimerism Study; Molecular
Methodology:PCR amplification of DNA and capillary gel electrophoresis
CPT Codes: 81265 x1, 81266 x1
Test Includes:BMT DNA processing, DNA marker pre BMT polymorphism.
Turnaround Time:Performed Mon-Fri; results are reported 1-2 weeks after both donor and pre-BMT recipient specimens have been received.
Special Instructions:Recipient sample only needs to be sent once. Duplicate recipient samples will be canceled. Record the transplant recipient's name on request form when sending the donor specimen. (Donor specimen is requested for dnenft testing.)

Collection Instructions

Specimen:Blood, bone marrow aspirate, bone marrow biopsy, tissue biopsy or buccal scrapings
Minimum\Peds Volume:Contact laboratory if unable to obtain specified collection volume.
Container:Yellow (ACD, Solution A) tube available from laboratory
Alternate Containers: Purple (EDTA)
Collection Instructions:

Peripheral blood is the ideal specimen for this test, unless patient has received bone marrow transplant previously.

·    Peripheral Blood: RECIPIENT: Collect 10 mL, 5 mL minimum in yellow ACD (Solution A) tube; alternate container: purple (EDTA) tube.

·    Bone marrow aspirate: Aspirate ACD (or EDTA) solution into syringe from open ACD (or EDTA) tube; collect 2-5 mL bone marrow into syringe. Deliver in syringe.

·   Tissue: 3 mm x 3 mm x 3 mm in sterile container; deliver to the laboratory within 1 hour, freeze specimen at -70 C, or use RPMI.

·   Buccal scrapings: Get swabs from MDL Lab (call 612-273-8445). Put in sterile saline.

Causes for Rejection:Frozen  or clotted specimen; incorrect anticoagulant. DNA extracted at non-CLIA certified (or equivalent) lab.
Shared samples will not be accepted unless sent to Molecular first. Contamination will occur on automated hematology analyzers.

Processing and Shipping

Specimen Processing:Store blood, bone marrow aspirate, and buccal scrapings at room temperature. Store tissue at -70C.
Shipping Instructions:Ship blood, bone marrow, or buccal scrapings at room temperature. Must arrive within 24 hours. Ship tissue at room temperature to arrive within 1 hour; alternatively, ship on dry ice to arrive within 24 hours, or send in RPMI at 4C.
Test Performed at or Referral Lab Molecular Diagnostics  (UMMC East Bank)

Reference Range:

Results are reported as "Found" to indicate that there is a polymorphism identified that will allow the lab to follow the DNA engraftment of the patient post-bone marrow transplant or "None" to indicate that there is not a polymorphism and the DNA engraftment of the patient cannot be followed post bone marrow transplant. The name of the donor is included to assure that the correct donor is linked to the recipient.

Use:Evaluation for identification of informative polymorphic markers which are used to determine engraftment status subsequent to bone marrow transplantation. An informative marker is defined as one that demonstrates unique identity for recipient and donor DNA.
